UPCPMT Exam is conducted by Pt. Deen Dayal Upadhayaya University (DDU) every year. It is a state level examination which is held for the candidates belonging to the state of Uttar Pradesh. The exam is held for the candidates who are interested in various medical courses offered by the government and private colleges of Uttar Pradesh. Application form is the first step for any exam. UPCPMT Application Form
UPCPMT application form will be available from third week of March 2016 tentatively. The candidates should fill the application form correctly and completely. Incomplete and incorrect information in the application form will lead to disqualification of the candidate.
How to fill Application Form
The candidates have to follow the following steps for filling the application form:
- Go to the official site.
- Login to UPCPMT application form. New candidates have to register and have to click on new user in which they have to fill basic details like name, address, email id and their mobile number.
- A verification code shall be sent to applicant’s mobile number.
- Candidates are required to carefully fill in all the details being asked in the online Application Form of UPCPMT 2016.
- After filling in all the information, candidates are required to click on the ‘Submit’ button.
- Application fee for UPCPMT 2016 can be paid either online or offline.
- Online Payment – Online payment can be done through credit and debit card.
- Offline payment – Offline payment can be done through State Bank of India Challan which can be downloaded from the official website.
- Declaration Form – Candidates have to download the declaration form and filled the required as per the given instructions. The photograph in declaration form must be attested by the gazetted officer or the principal of the respective colleges from where student is pursuing the higher secondary education. Candidates have to upload the scan copy of UPCPMT 2016 declaration form after pasting their photograph, index finger impression and sign of their parent/guardian.
- Email and SMS will be sent to candidates after successfully registration of UPCPMT 2015 application form.
- Candidates must take printout of UPCPMT 2016 application form for their future reference.
